HRiA completed a 22-month statewide assessment and planning process as the contracting organization for the Connecticut Department of Public Health on their State Health Assessment (SHA) and State Health Improvement Plan (SHIP), titled Healthy Connecticut 2020.  For the assessment, HRiA worked with hospitals, local health departments, professional associations, and community groups around the state to gather and analyze data on a range of health issues to provide a comprehensive portrait of the health of Connecticut residents, with a focus on health equity and disparities among specific sub-populations. HRiA then facilitated the state health improvement process which involved an intensive, collaborative structure of six working groups of stakeholders meeting over an 18-month period to identify priority health issues, goals, measurable objectives, and strategies to address needs so that Connecticut agencies and organizations could better coordinate and align efforts. HRiA is now providing technical support for the first year of SHIP implementation, monitoring, and reporting. These reports can be found at: http://www.ct.gov/dph/cwp/view.asp?a=3130&q=542346.
