HRiA conducted a comprehensive and collaborative Community Health Needs Assessment (CHNA) and Community Health Improvement Planning (CHIP) process for Somerset County, NJ. The project was spearheaded by the Robert Wood Johnson University Hospital-Somerset and the Healthier Somerset Coalition. A broadly representative stakeholder group of nearly 50 organizations including health department leaders, hospital representatives, and community-based organization leaders came together for this important assessment and planning work.
