HRiA facilitated a complex multi-sector, multi-stakeholder process in Mercer County, New Jersey, to develop the Greater Mercer Community Health Improvement Plan (CHIP) in 2012.  The Greater Mercer Public Health Partnership (GMPHP) developed with support from the Robert Wood Johnson Foundation.  The GMPHP is comprised of 14 nonprofit organizations. This includes 4 hospitals (Capital Health Medical Center- Hopewell, Princeton Health Care System, Robert Wood Johnson University Hospital-Hamilton, and St. Lawrence Rehabilitation Center), 9 local health departments, and the United Way of Greater Mercer County.

HRiA facilitated a rapid planning process over 2 half-days with a diverse group of participants including non-traditional public health stakeholders (i.e., students, transportation, and public safety).  HRiA worked to refine the output of the planning sessions to meet all national standards and requirements and to ensure a strong evidence-base for all proposed strategies. HRiA is now providing technical support for Year 1 Action Plan development, implementation, and reporting.
