Since 2016, HRiA has worked collaboratively with the Nicholson Foundation and the New Jersey Department of Health (NJDOH) to design and develop a pilot asthma home visiting program. The goals of the pilot are to improve asthma outcomes and reduce healthcare costs among children age two through 17 enrolled in NJ FamilyCare (Medicaid) by: (1) funding the implementation of an evidence-based, home-based asthma intervention; (2) supporting technical assistance (TA) and training for project staff; (3) funding an evaluation of post-intervention changes in patients’ asthma control and environmental conditions; and (4) partnering with the NJDOH to evaluate cost effectiveness for sustainability. In Phase 1, HRiA provided TA to the Foundation to: (1) organize and facilitate a convening of NJ stakeholders, including accountable care organizations and federally-qualified healthcare centers (FQHCs), Medicaid payers, and state officials to gather input for the pilot; (2) define the intervention, target populations, and core evaluation measures; (3) design the request for proposals; and (4) participate in review of the proposals and the selection of provider sites. In Phase 2 HRiA will: (1) assist sites with IRB applications, including protocols for secure data collection, storage, transfers, and destruction; (2) organize workforce development opportunities; (3) convene and facilitate a Providers Learning Community; (4) develop a tracking system and monitor site performance; (5) analyze data collected during the home intervention; and (6) engage policy makers and payer stakeholders.
